  • Manage clinical quality data and four data systems supporting the organization and nine external programs.

  • Prepare monthly, quarterly, and annual Quality Improvement Reports and Monthly Quality Presentations for care teams and the board of directors.

  • Create and manage data review processes to ensure accuracy and integrity of reports.

  • Assisted with quality data collection, analysis, and running reports for providers, committees, and leadership.

  • Primary liaison serving 28 providers through review of patient data and communication of care gaps.

  • Administrative support of the quality program through coordination of patient appointments, medications, and resources.

  • Assisted with monthly patient focus groups and conducted member calls and surveys to improve patient care and sentiment.

  • Lead patient communication, outreach and appointments for a hypertension grant responding to management of chronic diseases.
